Role: Sr. Java Fullstack Developer Location: Albany, New York Duration: 30 Months Job ID: SLG - 19 - 10301 Description Health Home Care Management Agencies (HHCMA) conduct the CANS-NY to determine health home acuity, as well as the HCBS LOC to determine HCBS eligibility on behalf of Health Homes. Health Homes also leverage this assessment data to develop plans of care. Currently data is only accessible to Health Homes via front end reporting and/or manual consumer record review. Implementing UPHN data exchange will enable Health Homes to receive, process, analyze and use this data in a more streamlined and timely manner. Given the patient population turnover, automated data exchange that operates on Health Home enrollment would ensure that the current Health Home would receive the consumer's relevant data, in support of the Health Home Serving Children Program's "record follows the person" approach to service delivery. Further, automated data exchange would enable Health Homes to monitor children/youth improvement more effectively. Duties The consultant will be performing application development tasks including researching and documenting technical designs, programming solutions, writing and executing unit test plans, researching, resolving, and testing defects, customer technical support, and close collaboration with other team members. Review business requirements documentation with technical manager to ensure clarity in implementation of technical design Implement systems and program design to meet highly complex business needs and technical design document. Creates appropriate documentation in work assignments such as program code and technical documentation. Assists technical manager in development time estimates and assigned tasks Codes, tests, debugs, implements, and documents highly complex programs, and developing complex test plans to verify logic programs. Troubleshoots complex end user technical issues, identifying problems and leading solutions Implements release management process including technical steps to promote, test and release through the development environment, coordinating with infrastructure staff. Uses iterative and agile software development life cycle methodologies. Mandatory Requirements Experience (12+Years) Is involved with all facets of the software development process, including the research, design, programming, testing and implementation of computer software.
